• Request a Recheck Appointment with Oncology: Please complete this form to request an Oncology recheck appointment. Because Oncology scheduling is so specialized we do not permit online booking for rechecks.
• Referring Veterinarian Form: Referring Veterinarians: use this link to refer a patient to one of our departments directly. Our staff will create a profile for them in our system, and our Specialty Coordinator will reach out accordingly. Please be sure to send records at the time of submission.
• Request a Prescription Refill: Existing TruVet clients may use this link for prescription refills to be picked up from our hospital, written prescriptions, or prescriptions shipped to your door directly through our online pharmacy service.
• New Patient Form: New to TruVet? Please complete this form prior to your first visit so we can have your pet's information ready in our system and make your check-in as smooth as possible.
• Book a Specialty Appointment: New patients seeking a specialty consultation in Cardiology, Internal Medicine, Oncology, or Surgery can conveniently request an appointment online through our Pet Owner Portal. Once your request has been submitted, a member of our team will contact you to confirm your appointment and collect the required deposit to reserve your visit. Please note that deposits are fully refundable if your appointment is canceled with at least 48 hours' notice.
• Book a Wellness Appointment: New to the area and/or do not have a pre-established relationship with a veterinarian? Join the TruVet Wellness and Urgent Care roster. New patients seeking a wellness consultation can conveniently request an appointment online through our Pet Owner Portal. Once your request has been submitted, a member of our team will contact you to confirm your appointment and collect the required deposit to reserve your visit. Please note that deposits are fully refundable if your appointment is canceled with at least 48 hours' notice.
• Request an Appointment: Use this link to request an appointment with us if you are not sure what department you need to see! Note: Emergency services are on a first come, first serve basis only. If you suspect your pet is experiencing a life threatening emergency, please take them to the nearest emergency room for assistance. When in doubt, don't wait it out!
