
What Is Distemper?
Canine distemper is a highly contagious viral disease that affects the respiratory, gastrointestinal, and nervous systems of dogs. Although it is highly preventable through vaccination, outbreaks still occur—most commonly in unvaccinated or under-vaccinated populations. Understanding how distemper spreads, how it progresses, and what outcomes to expect is critical for both pet owners and veterinary professionals.
Causes and Transmission of Canine Distemper Virus (CDV)
Canine distemper virus is an RNA virus closely related to measles in humans and rinderpest in cattle. It targets multiple body systems and spreads very efficiently.
Transmission occurs primarily through airborne respiratory droplets from coughing, sneezing, or barking; direct contact with infected bodily secretions such as saliva, urine, feces, or ocular and nasal discharge; and contaminated objects including food bowls, bedding, collars, or shared environments. In rare cases, the virus can be transmitted from mother to fetus via the placenta.
Importantly, infected dogs may begin shedding the virus before showing clinical signs and can continue shedding for several weeks, making early identification and isolation essential.
Signs and Symptoms: How Distemper Manifests
Clinical signs vary depending on the stage of infection and the systems involved.
Early and systemic signs reflect involvement of the respiratory and gastrointestinal tracts and may include fever and lethargy; thick nasal and eye discharge; coughing and sneezing; vomiting and diarrhea; loss of appetite and dehydration; and thickening or crusting of the nose and footpads, commonly referred to as “hard pad disease.”
Neurological Signs
As the disease progresses, the virus may invade the central nervous system, sometimes weeks to months after initial infection. Neurologic signs may include muscle twitching and involuntary movements; jaw spasms (“gum chewer’s syndrome”); seizures; circling, head tilt, or ataxia; and behavioral changes or disorientation. Once neurological involvement occurs, the prognosis worsens significantly, and deficits are often permanent.
Diagnosis
Diagnosis is based on a combination of PCR testing using blood, nasal, or ocular samples; serologic testing and bloodwork; imaging studies when indicated; and evaluation of clinical signs and patient history. Because distemper symptoms can overlap with other diseases, PCR testing is especially important for accurate diagnosis.
Treatment: Supportive, Not Curative
There is no antiviral cure for canine distemper. Treatment focuses on supportive care and management of complications. This may include IV fluids and nutritional support, antibiotics for secondary bacterial infections, anti-nausea and gastrointestinal medications, anti-seizure medications if neurologic signs develop, and oxygen therapy or hospitalization in severe cases. Because the virus suppresses the immune system, supportive care is critical but does not eliminate the virus itself.
Prognosis and Survival
Outcomes depend on age, immune status, and how quickly treatment begins. Adult dogs have an approximate mortality rate of 50% even with treatment, while puppies have a mortality rate that often exceeds 80%. In some wildlife species, fatality rates approach 100%. Once the virus affects the brain or spinal cord, prognosis becomes guarded to poor, even in survivors.
Long-Term and Lifelong Effects
Dogs that survive distemper may experience chronic or delayed complications, including persistent or recurring seizures; muscle tremors or twitching; cognitive dysfunction; vision impairment; and neurological deficits that may appear months or years later. While some symptoms can be managed with long-term medication, these complications can significantly impact quality of life and longevity.
Prevention: The Best Medicine
Vaccination is the most effective defense against canine distemper. Puppies should receive their first distemper vaccine between 6 and 8 weeks of age, with boosters every 3–4 weeks until 16–20 weeks. Adult dogs should receive boosters according to veterinary recommendations. Limiting exposure to unvaccinated animals and wildlife is essential, and maintaining high vaccination rates helps protect both domestic dogs and wildlife populations.
