Lately, we have been seeing a fair number of cases of Immune-Mediated Hemolytic Anemia (IMHA) so this month's topic is to ensure everyone knows and understands the disease. IMHA is one of the most challenging and critical conditions veterinary staff encounter in small animal practice. Early recognition and vigilant patient care can mean the difference between stabilization and crisis.

What Is IMHA?

IMHA occurs when the body's immune system mistakenly destroys its own red blood cells (RBCs). This can happen within the bloodstream (intravascular hemolysis) or in the spleen and liver (extravascular hemolysis). The result is severe anemia, decreased oxygen delivery, and, in many cases, a rapidly deteriorating patient.

There are two main types:

  • Primary (idiopathic) IMHA: The immune system attacks RBCs without an identifiable trigger.
  • Secondary IMHA: Triggered by an underlying cause such as infection, neoplasia (cancer), drugs, or vaccination.

Recognizing the Signs

Veterinary staff are often the first to spot subtle changes. Common clinical signs include:

  • Pale or icteric (yellow) mucous membranes
  • Tachycardia and tachypnea
  • Lethargy and weakness
  • Dark or red-tinged urine
  • Splenomegaly or hepatomegaly on palpation

PCV/hematocrit monitoring is crucial — a rapid drop is a red flag. Blood smears may reveal spherocytes or autoagglutination, hallmarks of IMHA.

Diagnostic Support

IMHA is typically confirmed through a combination of:

  • CBC: Regenerative anemia, reticulocytosis
  • Positive saline agglutination test or Coombs' test
  • Chemistry and urinalysis: To assess organ function and detect hemoglobinuria
  • Imaging: To rule out underlying causes such as neoplasia or infection

Treatment and Nursing Care

Management is intensive and often prolonged:

  • Immunosuppressive therapy: Prednisone is first-line; azathioprine, cyclosporine, or mycophenolate may be added.
  • Supportive care: IV fluids, oxygen therapy, gastroprotectants, and sometimes blood transfusions.
  • Antithrombotic therapy: Low-dose aspirin or clopidogrel to prevent life-threatening thromboembolic events.

Nursing care focuses on monitoring and minimizing stress:

  • Monitor PCV/TP at regular intervals.
  • Record respiratory effort, gum color, and mentation.
  • Handle patients gently to reduce oxygen demand.
  • Keep meticulous records — small changes can signal big shifts in patient status.
